Space Maintainer
A space maintainer is a removable or fixed dental appliance designed to hold open a space when a baby tooth is lost too early. Maintaining this space allows the permanent tooth to erupt into its proper position.
Purpose of a Space Maintainer
Space maintainers are most commonly used in children who lose baby teeth prematurely due to decay or injury. When a baby tooth is lost before the permanent tooth is ready to erupt, nearby teeth can shift or drift into the empty space. Teeth in the opposing jaw may also move up or down into the gap.
If the space is not preserved, there may not be enough room for the permanent tooth to come in correctly.
Surprisingly, some baby teeth are not replaced by adult teeth until ages 12–14, making space maintenance especially important.
Crowding
When teeth drift into an open space, crowding can occur. This may cause the permanent tooth to:
- Erupt crooked or lopsided
- Become partially blocked
- Fail to erupt properly
Crowding can also:
- Make teeth harder to clean, increasing the risk of tooth decay
- Make chewing less efficient and cause uneven wear
- Lead to more extensive orthodontic treatment later
A space maintainer helps prevent these problems by preserving the space and guiding proper eruption.
What Is a Space Maintainer Like?
A space maintainer is a small, custom-made metal appliance that is firmly attached in the mouth and removed once the permanent tooth erupts.
It often consists of:
- A metal band cemented to a tooth next to the space
- A wire loop or spring that extends across the gap to maintain the space
The appliance stabilizes surrounding teeth and prevents them from shifting.

Care of a Space Maintainer
To avoid damage and unnecessary replacement:
- Avoid sticky sweets, chewing gum, popcorn, and ice
- Do not tug or push on the appliance with fingers or tongue
- Brush and floss carefully to keep the appliance clean
- Pay special attention to cleaning around wires and bands, as food can easily become trapped

- Inspect the appliance regularly for any loosening or damage
- Keep regular 6-month check-up appointments so we can monitor oral health and determine when the appliance should be removed
